I am a retired physician who had four two year terms on the executive committee of a major metropolitan hospital system.
The fundamental problem of E.R. waits is the contract with the E.R. physicians group. The hospital desperately wants patients moved through quickly. The rest of the medical staff want their patients seen quickly (we would rather be called to see a patient at 9 pm than 2 am). To maximize their income, E.R. groups understaff. The solution: eliminate E.R. group contracts and replace with hospital employed E.R. physicians. The employer can set E.R. physician staffing for patient need, not maximal income of the senior partners in the E.R. group. And patients will beat down their door to be taken there.
